Niacin: Adverse Effects
There are a number of adverse effects associated with niacin as cholesterol-lowering therapy.
  • Flushing
    • Occurs particularly with the immediate-release formulation.
  • Elevations in Uric Acid
    • Can occur and should be closely monitored in patients who are predisposed to gout.
  • Hepatotoxicity
    • Occurs at higher doses and is more pronounced with the extended-release formulation than with the immediate-release formulation. Patients with gallbladder disease or a history of jaundice, liver disease, or peptic ulcer disease should be monitored closely.
  • Increases in Blood Glucose
    • Occurs in diabetic patients, who should be closely monitored. An adjustment in diet and/or hypoglycemic therapy may be necessary.
  • Hypophosphatemia
    • Occurs rarely.
  • Reduction in Platelets
    • Occurs rarely.
