hyperlipidemia notes

CHD risk equivalents:

Diabetes
Periferal Vascular disease
Aortic aneurism
>20% ten year risk of CHD from Framinham projection
CAD

-------------------------------

LDL goal < 70 known CAD and low HDL

In a patient with known coronary artery disease and the additional risk factor of low HDL-C, the recommended primary therapeutic target for elevated LDL-C is the optional value of <70 mg/dL.

------------------------------------

ATP III goals for different patients

patient                                LDL-C                     non-HDL-C

no CHD 0-1 risk factor       <160                       <190
no CHD 2 + risk factor       <130                       <160
CHD/CHD equivalent         <100                       <130
CVD+DM/MS/Cig/ACS    <70                         <100

MS metabolic syndrome
ACS acute coronary syndrome

------------------------------------

LDL particle number

goal <700 ?

Landmark clinical trials have demonstrated significantly stronger associations between nuclear magnetic resonance (NMR)-measured low-density lipoprotein (LDL) particle number and cardiovascular outcomes than is observed with the standard lipid panel, directly measured LDL cholesterol, and apolipoprotein B. Regardless of the population studied, the LDL particle number was consistently a stronger predictor of risk.
